Check the system log
Check for the Spam attack
Check the queue status
Check for the SMTP concurrency
Check for the in-coming mails - rate of incoming
Corrective action depending on previous analysis
 
Queue getting accumulated

Check the status of the queue with Qmailctl and qmHandle
Check for distinction of Local or remote queue
If remote queue check the mail marks or the delivery failures
If local , check for the log / or any changes in configuration
